4. REQUIRED DOCUMENTATION

Upload a certified colour copy (both sides) of the current ID or, a certified colour copy of the current passport or, a certified colour copy of driving licence (both sides). For joint accounts, a certified colour copy of the identity card of all persons. For beneficiaries different than the investor(s), a certified colour copy (both sides) of the current ID or, a certified colour copy of the current passport or, a certified colour copy of driving licence (both sides).

Proof of bank account ownership: Copy of a bank statement or copy of the digital bank statement (showing name and IBAN account number); or copy of the bank's confirmation in response to the person's request to open a bank account with the bank account number shown on the Personal Information form; or copy of a direct debit receipt together with a confirmation that there are no other bank account holders in addition to the one(s) mentioned on the receipt. Document should be current and no older than 3 months.

(If applicable) Proof that beneficiary different than the investor can redeem on behalf of the investor(s). e,g, Legalized/ notarized document that appoints the legal representatives of the state and the beneficiaries.

If you or the joint account holder are unable to provide the TIN, please attach proof of the nationality of another country, certificate of loss of U.S. citizenship, or provide a reasonable explanation of why you or the joint account holder do not have such a certificate despite renouncing U.S. citizenship or the reason you or the joint account holder did not obtain U.S. citizenship at birth.
